Services are delivered at no cost to eligible members through the Social Care Network. A screening establishes which of these someone qualifies for — and Loom tracks each one through to delivery.

Medically tailored meals Prepared meals matched to a member's clinical and dietary needs, delivered to the home on a set schedule.
Nutritional assessment A professional reviews diet, conditions and the practical barriers in the way of eating well.
SNAP assistance Help applying for food benefits, and help keeping them at recertification — where most people lose them.
Cooking & nutrition education Practical sessions on preparing food that fits a member's health needs, budget and kitchen.
Utility assistance Support with heat and electricity costs, so a household is not choosing between the bill and food.
Registration assistance Help with enrolment and paperwork across programmes, in English or Spanish.
